I'm receiving the following error when viewing the admin/appearance/settings/md_springfield page:
Notice: Undefined index: icon in md_springfield_theme_settings_design() (line 781 of /var/www/<my-site>/sites/all/themes/md_springfield/inc/admin/theme-settings-design.inc)
No checkbox icons are showing up:

We will check this. There might some pproblems in installing the theme
While you're poking around on my site, I also see this error when viewing a Simplr Page Builder page:
Notice: Undefined variable: file in include() (line 8 of /var/www/<my-site>/sites/all/modules/contrib/springfield_config/templates/google-map.tpl.php).
