Red alert message on site

Hi Cindy

Can you advise me as to how to fix this issue. I am getting a red alert messsage on the front of my site:

Error message
Notice: Undefined index: #title in md_max_textfield() (line 14 of /home/users/uks81487/html/karen-rogers.co.uk/sites/all/themes/md_max/theme/textfield.theme.inc).
