Custom Block template

I created a custom block with a contact form and placed it in pages. Inside node page, to form is loosing its template. Please advice

Please open file md_alpine.theme, line 275 and replace

if($form['#form_id'] == "webform_submission_contact_form_add_form") {

with:

if($form['#form_id'] == "webform_submission_contact_form_add_form" || $form['#form_id'] == "webform_submission_contact_form_user_1_add_form") {

Save and clear Drupal cache!
