Table of Content menu showing + submenus not going full width

Hello:

Having many problems with MegaMenu.

1. I have a menu appearing called "Table of Content" which I can't seem to get rid of and I didn't create. Please let me know how to remove.

2. The sub-menus are not expanding horizontally - I am using the 4 column layout and even though I select show full width everything is squished together and not expanding horizontally.

I am running a fully updated version of Drupal 7 and have updated my jQuery per your video.
