I made my own webform with extra fields an get a list of the same errors (per field?)

(regel 353 van /home/shape/public_html/sites/all/modules/custom/md_themesetting/md_themesetting.module).
Can you explain me how to slove this?

Could you tell us how did you create your new fields and send us some screenshots? We would like to know which type of fields that you created in webform. Did you create a new node or add new fields in old node?
