When I add or edit an HTML item, the pop-up window just has a title line (Edit HTML Code), blank main section and the Apply/Cancel/Remove buttons.
No error appears in the log file.
What might cause the html editor control to not appear?

That can't be right. It's a type of thing you can drag to a menu and I found at least one other support note about it: http://megadrupal.com/forum/module-support/mega-menu/301

We now do not support HTML Editor anymore. You just can directly text HTML
