Please do following steps:

1. Open template.php file in md-alpine/sites/all/themes/md_alpine. Then go to line 217 you will see this code

$form['actions']['submit']['#suffix'] = '</span></div></div></div>';

Delete it and replace by

$form['actions']['submit']['#prefix'] = '<div class="col-md-12 text-center"><div class="action mybutton medium"><span>';
$form['actions']['submit']['#suffix'] = '</span></div></div><br />';

2. Open style.css file(sites/all/themes/md_alpine/css). Scroll down to the bottom and you will see
